Home Health Care

Improve your or your loved one’s well-being and independence with home care from UNC Health Johnston. We provide medically necessary services to treat, rehabilitate, sustain or restore homebound adults to their optimal health in the setting where they feel most comfortable.

Patients qualify for services if they are homebound and/or have a medical need that requires the skilled care of a clinician. On-call services are available evenings and weekends, as needed.

Who Needs Home Care?

Home health care helps people 18 and older who have trouble leaving home due to:

  • Disability
  • Recovery from surgery, a medical condition, or an injury
  • Weakness

A doctor must certify that you or a family member needs home care. Call our office any time for guidance.

Health Care Services

A nurse will work with you or your loved one, along with family caregivers, to make a personalized care plan. It may include home health services such as:

  • Disease management
  • Education on medical conditions
  • Hospice care for terminal illnesses
  • Intravenous (IV) therapy
  • Medication management
  • Rehabilitation, including occupational therapy, physical therapy, and speech therapy
  • Tube feedings and other specialty services
  • Urinary (Foley) catheter care
  • Wound care
  • Social worker evaluations and referrals for psychosocial care, community services, financial assistance, and other resources as needed

To arrange assistance with household chores, ask us for a referral to a personal care agency.

Home Therapy

Our physical and occupational therapist provide to you medically based evaluations and treatment techniques that are designed specifically for you to regain the function you lost whether the cause was from a hospitalization, rehabilitation facility or surgery.

Our clinical team is made up of highly trained physical and occupational therapists that have years of home care experience and utilize evidence-based techniques to treat your functional limitations. Experienced Staff

As often as possible, the same nurse or therapist will provide care. All home care nurses and therapists have years of experience and take time to get to know your family’s needs and preferences.

Insurance Covers Home Care

Medicare, Medicaid, and most private insurance plans cover home health care for homebound patients with a medical need. Ask us for help meeting your plan’s requirements for insurance coverage.
